This Application Form must be filled in by every individual applicant or a group representative (where applicable) who agrees to be bound by the Privacy Policy, Terms of Use, and Terms and Conditions for Competition Submissions.  Each applicant and group gives Catapult Advertising Pte Ltd permission to supply this information to the press.
